If you are Looking to apply to medical school, you are likely already well aware of the fierce level of competition for precious spots. Yes, one summer in Mexico, and one summer in Spain for college credit through Texas Tech University, Dept. of Classical and Modern Languages and Literatures.
However, if you are not interested in college credit, I will tell you some good things to look for. Make sure you get a program that does home stay. Make sure that they guarantee that nobody in the household speaks English. I had a friend who ended up spending a summer tutoring the family's kids in English which would have been fine if they hadn't been charging him room and board to help him with his Spanish. If they put you in a dorm with a bunch of other foreigners, then you will only speak English with your classmates. Make sure that you know if meals, sheets, and laundry are included. Two meals a day and laundry are pretty standard, but you want to make sure. Also make sure that is a limit on how many students stay in the house. You don't want to stay in a house with six English speakers. Two is the usual max, but if you feel confident, being the only student is better, although that is hard to find.
Look for schools that are a little off the beaten path. If you go to a language school at a beach resort, everyone speaks English, and again you won't get as much out of it.
The more you know before you go, the more you will learn while you are there. My college required that we have at least four semesters before we went and I came back fluent after five weeks. I had a friend who went for an entire summer without having taken a class beforehand and came back only knowing how to say, "Good evening, I would like a glass of red wine."
Also the longer you stay the better. Anything less than two weeks is a waste of time.
